How do anti-wrinkle injections work
Botulinum toxin works by temporarily blocking the signal between nerve and muscle, so the treated muscle contracts less strongly. Because many facial lines are created by repeated movement, softening that movement allows the overlying skin to smooth. It treats dynamic lines, meaning those caused by expression, and the most commonly treated areas are the forehead, the frown lines between the brows and the lines at the outer corners of the eyes.
Effects are not immediate. Most people notice change from around three to five days, with the full effect at about two weeks, and the result typically lasts three to four months before movement gradually returns. It is worth understanding what it does not do. Deep lines that are visible at rest, volume loss and skin quality are different problems needing different treatments, and a good assessment will tell you honestly which of these is driving what you see in the mirror.
The UK law patients are rarely told about
Botulinum toxin is a prescription-only medicine under the Human Medicines Regulations 2012, so it can only be supplied or administered in accordance with a prescription written for you as an individual. That prescription must follow a face-to-face assessment, because remote prescribing of injectable cosmetic medicines is not accepted by the UK regulators, a position the GMC set out in 2012 and which has since been reinforced across the other regulators, including for nursing and midwifery prescribers from June 2025.
There is also a clear age rule. Under the Botulinum Toxin and Cosmetic Fillers (Children) Act 2021 it is a criminal offence in England to administer botulinum toxin or dermal filler for cosmetic purposes to anyone under 18, and an offence to arrange it. Separately, PRP and botulinum toxin both sit in the medium-risk amber tier of England's forthcoming licensing scheme, under which qualified healthcare professionals meeting agreed standards may perform independently while non-healthcare practitioners would require oversight from a named regulated healthcare professional.

Why product provenance matters
This is the safety issue the sector has been forced to confront. Between June and August 2025, 41 confirmed cases of botulism in England were linked to unlicensed botulinum toxin products, and a BBC investigation broadcast in September 2025 documented prescriptions being issued without face-to-face consultations and unlicensed products being supplied. Botulism from injected toxin is a serious illness, and these cases were entirely avoidable.
The lesson for patients is practical. Ask what product is being used and whether it is licensed for use in the UK, ask who prescribed it and whether they assessed you in person, and be sceptical of prices far below the market or of treatment offered in a non-clinical setting. Licensed product handled correctly requires cold-chain storage and a proper supply route through a UK-regulated pharmacy. What to expect, and the risks
Treatment itself is quick, usually taking around ten to fifteen minutes, using very fine needles, and most people describe it as a series of small pinches. There is little downtime, though you will normally be advised to stay upright for a few hours, avoid rubbing the area, and avoid strenuous exercise, saunas and heavy alcohol on the day.
Common short-lived effects include small red marks at injection points, mild bruising and occasionally a temporary headache. Less common effects include a heavy-feeling brow or, rarely, temporary drooping of the brow or eyelid, which occurs when the product affects a muscle beyond the intended target and which settles as the effect wears off. Correct dosing and placement based on your individual anatomy are what reduce this risk, which is a matter of medical training rather than product. Treatment is avoided in pregnancy and breastfeeding, in certain neuromuscular conditions and where there is infection at the site, all of which a proper medical history identifies.
