Stem Cell Knee Injections UK. Cost, Evidence And Better-Value Options

Stem cell knee injections are widely advertised in the UK, typically costing £3,000 to £7,000 for one knee. The largest randomised trial to date, MILES, published in Nature Medicine, compared 3 types of cell injection with a standard steroid injection in 480 people with knee arthritis. At 1 year, none of the cell injections was better than steroid for pain, and none changed the knee on MRI. NICE describes stem cell injections for osteoarthritis as experimental and best used in research. For most people, there are better-value options with stronger evidence.

Key points

  • In the MILES trial of 480 patients, bone marrow concentrate, fat-derived cells and umbilical cord cells were no better than a steroid injection for knee pain at 1 year, and none was better than the others.

  • UK prices commonly run from around £3,000 to £7,000 for 1 knee, with bone marrow concentrate listed from around £3,000 in London and up to £6,500 as an inpatient procedure.

  • Many so-called stem cell treatments contain few true stem cells, a point the British Orthopaedic Association has raised.

  • At The London PRP Clinic by The Wellness, ultrasound-guided knee PRP is from £595 to £995 per treatment, with a course of 3 priced at 10 percent off booking individually.

What stem cell knee injections are

The term covers several different treatments.

Bone marrow concentrate

Bone marrow is drawn from the pelvis under local anaesthetic or sedation, concentrated and injected into the knee. It is often called BMAC.

Fat-derived cells

Fat is removed by liposuction, processed and injected. Products are sometimes called stromal vascular fraction or by brand names.

Donor cells

Cells from donated umbilical cord or placental tissue, processed by a laboratory.

All 3 contain a mix of cells, and the true stem cell content is usually small. The theory is that the cells release signals that reduce inflammation and support repair, rather than turning into new cartilage.

What the evidence shows

The MILES trial

The MILES trial, led from Emory University and published in Nature Medicine in 2023, randomised 480 people with knee arthritis to bone marrow concentrate, fat-derived cells, umbilical cord cells or a steroid injection. At 1 year, pain and function improved in every group, but none of the cell injections was better than steroid, and none was better than the others. MRI showed no meaningful change in the knee in any group, and there were no serious procedure-related side effects. One of the investigators noted the conflict of interest when clinics charge high prices for a treatment that has not shown clear benefit over steroid.

NICE

NICE describes intra-articular stem cell injection for osteoarthritis as an experimental treatment that should be used in research rather than routine practice.

What this means

Stem cell injections appear safe in trials, and people do feel better after them, but so do people after a much cheaper steroid injection. There is no good evidence that they regrow cartilage. Promising laboratory work, such as the 2026 Stanford cartilage study, is covered in new knee arthritis treatments 2026.

What it costs

Indicative UK self-pay prices for 1 knee commonly run from around £3,000 to £7,000. London listings include bone marrow concentrate from around £3,000, and from £6,500 as a standalone inpatient procedure. Fat-derived procedures are often £4,000 to £8,000 or more.

For comparison, Arthrosamid typically costs £2,000 to £3,500 per knee, and specialist London clinics list knee PRP from £660 to £1,095 per injection.

Questions to ask any stem cell clinic

What exactly is being injected, and how many cells does it contain? Is the product regulated, and by whom? What randomised trial evidence supports it for my condition? How does it compare with steroid, hyaluronic acid or PRP? What does the full price include, from consultation to follow-up? Who performs the procedure, and is it image guided?

Better-value options with evidence

PRP

PRP has the largest trial base of the regenerative knee injections. A 2025 network meta-analysis of 10 randomised trials found a course of 3 injections outperformed a single injection at 1, 3 and 6 months, and meta-analyses favour PRP over hyaluronic acid. A 2021 JAMA trial found PRP no better than placebo, so the evidence is favourable but contested. See how many PRP injections for the knee.

Arthrosamid

A permanent gel given once, as effective as hyaluronic acid in a 2024 trial of 239 patients. See Arthrosamid vs PRP.

The foundations

Strengthening exercise and weight management remain the most effective long-term treatments, and every option is compared in knee injections London. Severe arthritis needs a surgical opinion, as explained in too young for a knee replacement.

What happens at The London PRP Clinic by The Wellness

You are assessed by a doctor who examines the knee, hip and the way you walk, and reviews the knee with ultrasound. X-ray or MRI is arranged at the London centre best equipped for that question if needed. You then receive a written recommendation across every option, including those we do not provide, with the cost of each, so you can decide with the full picture. The evidence across conditions is in PRP success rates.

What it costs here

At The London PRP Clinic by The Wellness, there are 3 ways to begin.

The diagnostic package, from £695, covers assessment, imaging and blood tests, and gives you a written recommendation whatever you decide.

A single ultrasound-guided knee PRP injection is from £595 to £995.

A course of 3 injections is priced at 10 percent off booking individually. More detail is in joint PRP cost in London.

Why people choose The London PRP Clinic by The Wellness

Every treatment is performed by a GMC-registered doctor, and no clinician treats a patient here until they have completed 100 supervised treatments. Every knee injection is ultrasound guided. We publish where treatments work and where they do not, and we compare every option at assessment so you do not pay for a treatment the evidence does not support. We see patients at 10 Portman Square in Marylebone and the Light Centre in Belgravia, and report an 87 percent patient success rate and more than 187 five-star reviews.

Frequently asked questions about stem cell knee injections

Do stem cell injections work for knee arthritis?

The largest trial, MILES, found that bone marrow, fat-derived and umbilical cord cell injections were no better than a steroid injection for knee pain at 1 year.

How much do stem cell knee injections cost in the UK?

Commonly around £3,000 to £7,000 for 1 knee, with bone marrow concentrate listed from around £3,000 in London and up to £6,500 as an inpatient procedure.

Can stem cells regrow knee cartilage?

There is no good evidence that current stem cell injections regrow cartilage in people. MRI showed no meaningful change in the MILES trial.

What does NICE say about stem cell injections for arthritis?

NICE describes intra-articular stem cell injection for osteoarthritis as experimental and best used in research.

Is PRP better than stem cells for the knee?

They have not been compared directly in large trials. PRP has a larger evidence base and costs considerably less.

What should I ask a stem cell clinic?

What is being injected, how many cells it contains, how it is regulated, what trial evidence supports it, how it compares with cheaper options, and what the full price includes.
