Deuruxolitinib On The NHS. Who Qualifies For The New Alopecia Areata Tablet

Deuruxolitinib, sold as Leqselvi, is a twice-daily tablet for severe alopecia areata. NICE recommended it for adults in England in July 2026, making it the second JAK inhibitor available on the NHS for this condition after ritlecitinib. In its main trial, about 1 in 3 adults on the 8 mg dose had 80 percent or more of their scalp hair back after 24 weeks, compared with fewer than 1 in 100 on placebo. It is prescribed by specialist dermatology teams, and it is for severe disease only. People with patchy alopecia areata have other effective options.

Key points

  • NICE recommended deuruxolitinib for adults with severe alopecia areata in guidance TA1178, published 15 July 2026, with NHS access expected from mid-August 2026.

  • In the THRIVE-AA2 trial of 517 adults, published in 2026, 33 percent on 8 mg twice daily reached 20 percent or less scalp hair loss at 24 weeks, against 0.8 percent on placebo.

  • Like other JAK inhibitors, it needs monitoring with blood tests and carries warnings about infections and blood clots. It is started by consultant dermatologists.

  • At The London PRP Clinic by The Wellness, a hair consultation with trichoscopy is from Β£300 and a blood panel through The Online GP by The Wellness is from Β£295, to confirm the diagnosis and support a referral.

What deuruxolitinib is

Alopecia areata is an autoimmune condition in which the immune system attacks hair follicles, causing round bald patches that can spread to the whole scalp, called alopecia totalis, or the whole body, called alopecia universalis. The follicles are not destroyed, which is why hair can regrow.

Deuruxolitinib is a JAK inhibitor. It blocks JAK1 and JAK2, signalling proteins that drive the immune attack on the follicle. By calming that signal, it allows follicles to resume growth. It is taken as a tablet twice a day, and the MHRA approved it for severe alopecia areata in adults on 12 March 2026.

Who qualifies on the NHS

NICE recommends deuruxolitinib for adults aged 18 and over with severe alopecia areata. In practice, severe disease is usually judged with the SALT score, which estimates the percentage of scalp hair lost, with severe often meaning 50 percent or more, and includes alopecia totalis and universalis. The decision is made by an NHS specialist dermatology team, which will consider other treatments, your medical history and the risks.

How it fits with ritlecitinib

Ritlecitinib, sold as Litfulo, was recommended by NICE in 2024 for severe alopecia areata in people aged 12 and over. Deuruxolitinib gives adults a second option. Your dermatologist will advise which suits you.

How to access it

Ask your GP for a referral to NHS dermatology if you have extensive alopecia areata. NHS organisations in England must make NICE-recommended treatments available within a set period of the guidance, but local formularies and waiting times can delay access. Private consultant dermatologists can also prescribe JAK inhibitors, with ongoing monitoring.

What the trials showed

The THRIVE-AA2 trial, published by Tsianakas and colleagues in the Journal of the American Academy of Dermatology in 2026, randomised 517 adults with severe alopecia areata to deuruxolitinib 8 mg twice daily, 12 mg twice daily or placebo for 24 weeks. The main measure was a SALT score of 20 or less, meaning 80 percent or more of scalp hair present.

At 24 weeks, 33.0 percent reached that target on 8 mg and 38.3 percent on 12 mg, compared with 0.8 percent on placebo. The 8 mg dose is the one approved for use. Many people who did not reach the target still had meaningful regrowth, and response often continues to build beyond 24 weeks.

Side effects and safety

In THRIVE-AA2, the most common side effects included infections such as COVID-19 and colds, headache, acne and a rise in a muscle enzyme called creatine phosphokinase. Serious side effects affected around 1 to 2 percent. No blood clots, heart attacks or strokes occurred in the first 24 weeks, but blood clots were seen in some people in longer extension studies. JAK inhibitors as a class carry warnings about serious infections, blood clots, cardiovascular events and some cancers, particularly in older people and smokers. They are not suitable in pregnancy. Regular blood tests are needed.

If treatment is stopped, hair loss often returns, so it is usually a long-term decision taken with your dermatologist.

What helps if you do not qualify

Most people with alopecia areata have patchy disease, which is not treated with JAK inhibitors on the NHS. For limited patches, options include steroid injections into the patches, strong steroid creams or lotions, and in some cases PRP, which small trials suggest may help patchy alopecia areata. Many small patches regrow on their own within a year.

Alopecia areata is more common in people with autoimmune thyroid disease and has been linked with low vitamin D, so blood tests are worthwhile. See thyroid hair loss and vitamin D and hair loss. Our full treatment guide is alopecia areata treatment.

How we help

You are assessed with a full scalp examination and trichoscopy, which confirms alopecia areata and separates it from other causes of patchy hair loss, such as scarring alopecias or fungal infection. A blood panel arranged by The Online GP by The Wellness and processed by an accredited partner laboratory checks thyroid function with antibodies, vitamin D, ferritin, B12, zinc and a full blood count. See the hair loss blood panel.

If your disease is severe, we write a clear referral letter with your SALT score, photographs and results, so your dermatologist has what they need. If your disease is patchy, we discuss the treatments that suit it, including PRP where appropriate.
