How women's hair loss differs from men's

Men with pattern hair loss usually lose hair at the temples and crown. Women more often keep their hairline and thin across the top of the head, so the parting widens and more scalp shows under bright light. The Ludwig scale describes this pattern. The bigger difference is cause. Women's hair responds to changes in iron stores, thyroid function, hormones, weight and stress, and several of these often overlap. A woman with early female pattern hair loss who also has low ferritin and a recent period of heavy shedding needs all 3 addressed. Treating only 1 is a common reason treatment seems not to work.

The causes we look for

Female pattern hair loss, where follicles gradually shrink under hormonal and genetic influence.

Telogen effluvium, a sudden increase in shedding 2 to 3 months after a trigger such as illness, surgery, childbirth, crash dieting or severe stress. It usually recovers once the trigger resolves.

Iron deficiency and thyroid disease, both common and both treatable.

Scarring alopecias such as frontal fibrosing alopecia, which destroy follicles permanently and need a dermatologist quickly. PRP alone is not the treatment for these.

What the evidence shows

PRP in women

The 2024 meta-analysis by Yuan and colleagues pooled randomised controlled trials of PRP in women with hair loss. Compared with control treatment, PRP reduced the number of hairs removed on a hair pull test and improved hair thickness and density. The effect on total hair count was less consistent between studies. That pattern fits what we see in clinic. PRP tends to strengthen and thicken existing hair and reduce shedding, rather than restore hair where follicles have gone.

PRP with minoxidil

Topical minoxidil is licensed for female pattern hair loss in the UK and remains a first-line treatment. PRP and minoxidil work in different ways, and many doctors combine them. PRP is a reasonable choice if

You have female pattern hair loss confirmed on examination, with follicles still present. You have corrected, or are correcting, any deficiency. You want to add to minoxidil, or you cannot tolerate it. You are prepared for a course of 3 sessions and a review at 6 months, because hair responds slowly.

Another route is usually better if

Your shedding started suddenly 2 to 3 months after a clear trigger, in which case it often recovers on its own once the cause is treated. You have signs of scarring alopecia, such as redness, scale or shiny patches around follicles. You are pregnant or breastfeeding, when many hair loss medicines are not suitable and timing is best discussed with your doctor.
