Hair Loss After Weight Loss Surgery

Hair shedding in the months after a gastric sleeve, bypass or other bariatric procedure is common, and for most people it is temporary. Two things drive it. The physical stress of surgery and rapid weight loss pushes many hairs into the resting phase at once, and the nutritional changes that follow bariatric surgery can leave the follicle short of protein, iron, zinc and B12. The first resolves with time. The second needs finding and correcting.

Why it happens

The surgical shed

Hair grows in cycles, with most follicles in a long growing phase and a smaller proportion resting for around 100 days before the hair releases. A major physical stress, such as surgery, anaesthesia, and the rapid weight loss that follows a bariatric procedure, can push a large group of follicles into the resting phase at the same time. Around 3 to 6 months later those hairs release together.

The result is diffuse shedding across the whole scalp, arriving at a point when many people are otherwise feeling well and pleased with their progress, which makes it all the more distressing.

The nutritional component

This is what distinguishes hair loss after bariatric surgery from shedding after other operations. Procedures such as sleeve gastrectomy and gastric bypass reduce how much you eat and, in the case of bypass, change where nutrients are absorbed. Protein intake is frequently lower than the body needs in the early months, and deficiencies of iron, zinc, vitamin B12, folate and vitamin D are recognised risks.

Hair is one of the most metabolically demanding structures in the body and one of the first to be deprioritised when nutrients are short. A follicle lacking protein or iron cannot sustain normal growth, so an uncorrected deficiency can turn a temporary shed into a prolonged one.

How it differs from GLP-1 medication hair loss

Weight-loss medications such as semaglutide and tirzepatide can also trigger shedding, largely through the same rapid weight loss mechanism. The difference after surgery is the added nutritional absorption change, which makes deficiency more likely and monitoring more important. The principles of assessment are the same in both.

Protecting your hair after surgery

Free, practical and mostly within your control.

Hit your protein target. Your bariatric team will have given you one, and it matters for hair as much as for muscle. Low protein intake is one of the most common contributors to prolonged shedding after surgery.

Take the supplements your bariatric team prescribes, every day, for as long as they advise. Lifelong supplementation is standard after many bariatric procedures, and stopping is a frequent route to deficiency.

Keep your follow-up blood tests. Bariatric programmes monitor nutritional status for good reason, and those results are useful for your hair as well.

Handle your hair gently during the shedding phase, with less heat and tension, and take monthly photographs of your parting in the same light so you can see recovery rather than guess at it.

Do not start an expensive hair treatment in the first few months. If the shed is post-surgical and your nutrition is corrected, it will recover, and you will otherwise attribute that recovery to whatever you bought.

What to test

Full iron studies including ferritin, since iron deficiency is common after bariatric surgery and a result just above the lower limit is frequently reported as normal while being too low to support good hair growth.

Vitamin B12 and folate, zinc and vitamin D.

Full blood count, albumin as a marker of protein status, full thyroid function and HbA1c.

Many of these will already be part of your bariatric follow-up. Bring your results. Reading them against what hair needs, rather than against what excludes deficiency disease, is often the most useful thing we do for people in this situation.

When it is more than a post-surgical shed

Shedding that continues well beyond the first year after surgery, or keeps recurring.

Thinning concentrated at the parting, the crown or the hairline rather than spread across the whole scalp.

Hairs coming through finer and shorter rather than full-length, which suggests miniaturisation rather than shedding.

Any scalp symptoms such as redness, scaling, itching or tenderness.

These suggest either an ongoing deficiency or an underlying pattern hair loss that the shed has revealed or accelerated, and both are worth assessing.

Where treatment fits

For a straightforward post-surgical shed, the treatment is nutritional correction and time. For pattern thinning that has been revealed or accelerated, treatment helps and earlier is better, since follicles respond while they are shrinking rather than once they have closed.
