Hair Loss In Your 20s. What Young Men Should Do First
Hair loss in your 20s is common. About 25 percent of men notice the first signs before the age of 21. In most young men it is male pattern hair loss, which is gradual and progressive but very treatable when it is caught early, because follicles that are shrinking can still be kept. The first step is a proper diagnosis with a scalp examination and blood tests, followed by a plan that you can keep up for years.

Why hair thins in your 20s
Male pattern hair loss
Most hair loss in young men is androgenetic alopecia, also called male pattern hair loss. Follicles that are genetically sensitive to dihydrotestosterone, a hormone made from testosterone, gradually shrink. Each new hair grows a little finer and shorter until the follicle stops producing visible hair. It usually starts at the temples, the crown or both, and doctors stage it on the Norwood scale, which we explain in the Norwood and Ludwig scales.
The process is slow, which is the good news. Shrinking follicles are still alive for years, and treatment can hold them or partly reverse the shrinkage.
Other causes worth ruling out
A sudden increase in shedding, rather than gradual thinning, often has a different cause. Common ones in young men include a recent illness, rapid weight loss, very restrictive diets, severe stress and low iron or vitamin D. Anabolic steroids and testosterone replacement can speed up pattern loss, which we cover in TRT and hair loss. Scalp conditions such as seborrhoeic dermatitis can add to shedding. Patchy, round bald areas suggest alopecia areata, which is treated differently.
Why acting early matters
Every treatment for pattern hair loss works best on follicles that are still producing hair. A man in his mid 20s with early thinning at the temples usually has far more to keep than a man in his late 30s with a smooth crown. Hair loss that starts young also tends to progress further over a lifetime if it is left alone. Starting a plan early is the single most effective decision most young men can make, and it is also the least expensive, because keeping hair costs much less than replacing it.

What works, and what the evidence shows
Finasteride
Finasteride tablets lower dihydrotestosterone and remain the most effective medical treatment for male pattern hair loss. The original trials of more than 1,500 men showed it slowed or stopped hair loss in most men over 5 years. The MHRA has since strengthened its warnings. Finasteride is associated with depression, suicidal thoughts and sexual dysfunction that may persist after treatment stops, and every pack now carries a patient card. Men taking finasteride 1 mg who develop low mood or suicidal thoughts should stop it and seek help urgently. Minoxidil
Topical minoxidil prolongs the growth phase of the hair cycle and is available without prescription. Low-dose oral minoxidil is increasingly prescribed off-label by doctors experienced in hair loss, after assessment, as we explain in oral minoxidil. An early increase in shedding is common and usually temporary.
PRP
PRP uses growth factors from your own blood to support follicles. A 2025 meta-analysis of 43 randomised trials and 1,877 participants found it improved hair density in pattern hair loss. It does not act on hormones, which makes it a valuable option for men who prefer not to take finasteride, and it can be combined with minoxidil. What about a hair transplant?
A transplant moves follicles from the back of the head to thinning areas. It does not stop the hair around those grafts from continuing to thin. In your 20s, the final pattern of your hair loss is not yet clear, which is why many hair surgeons advise stabilising the hair medically first.
