PRP in both knees at once: convenience is not the only question
Information checked 24 October 2026. General information, not individual medical advice.
Can I have PRP in both knees at the same appointment?
It may be considered in selected circumstances, but there is not strong evidence that one visit is the best approach for everyone. Each knee needs an assessment, and the practical effect of having two sore knees matters. Do not assume a bilateral treatment is appropriate merely because both sides hurt or because one visit is more convenient. [1,2]
What the published consensus actually says
A 2020 French-speaking expert consensus considered treating symptomatic osteoarthritis in both knees at the same time. Most experts accepted the possibility, but the recommendation was classified as uncertain because agreement was insufficient. The authors explicitly said that the literature did not provide evidence to justify either position. This is a low-evidence practical discussion, not a comparative trial proving that same-day treatment is better. [1]
Why the day afterwards belongs in the decision
The experts highlighted a possible painful episode or inflammatory flare after injection, which could be a major functional limitation if both knees are affected. Before choosing one or two visits, discuss walking, stairs, getting home, work and support at home. Ask for individual driving and activity advice. Do not plan around a promise of no downtime. [1]

Two knees may need two different plans
Pain on both sides does not establish the same diagnosis or severity. Bring existing imaging reports, previous treatments and the activities each knee limits. Ask whether the proposed treatment is addressing knee osteoarthritis or another problem. A plan should state why PRP is being considered for each knee rather than treating the second side as an automatic add-on.
The wider PRP evidence still applies
NICE says the evidence for knee PRP raises no major safety concerns but that efficacy evidence is limited in quality. It calls for special arrangements for governance, consent and audit or research. Treating two joints together does not remove that uncertainty or establish cartilage regrowth. Discuss possible benefits, risks and alternatives for your diagnosis. [2]
Questions to settle before paying
Ask whether the team recommends simultaneous or staged treatment and why; how the preparation and injection plan differ; what the total price covers; whether review is per knee or for the whole plan; and what happens if only one knee improves. Do not multiply a single-joint price or assume a package covers both. Obtain a confirmed written quote.

When a routine injection enquiry is the wrong next step
A newly hot, very swollen joint or sudden severe deterioration needs medical assessment, not a decision about a bilateral package. If a procedure has already happened, follow the treating team's safety and contact instructions and seek urgent help for concerning symptoms.
Choose the plan around your mobility
Contact The London PRP Clinic by The Wellness for an assessment of both knees. Explain your diagnosis if known, your previous treatment and what support you would have after the appointment. The useful outcome is a clear clinical and practical plan, whether that means one knee, staged treatment, a different option or no injection.
