Why combinations work

Hair loss has several drivers. In pattern hair loss, follicles shrink under hormonal influence, the growth phase shortens and blood supply to the follicle reduces. A single treatment usually acts on 1 of these. Combining treatments that act on different drivers gives each a better chance of working and, in trials, more hair overall. The principle is the same as in many areas of medicine, where 2 treatments with different mechanisms often outperform a higher dose of 1.

The combinations with evidence

PRP with minoxidil

Minoxidil lengthens the growth phase and improves blood flow to the follicle. PRP supplies growth factors that support the follicle and the cells around it. In the 2024 meta-analysis by Yao and colleagues, 5 randomised trials compared PRP plus topical minoxidil with minoxidil alone. Hair density was higher with the combination at every time point, by around 11 hairs per square centimetre at 1 month and around 18 at 6 months. The authors rated the evidence as low certainty and called for larger trials, which is a fair reading. In practice, it is the combination we recommend most often. See PRP vs minoxidil.

Microneedling with minoxidil

In the 2013 trial by Dhurat and colleagues, men using weekly microneedling with minoxidil grew about 4 times more hair than men using minoxidil alone at 12 weeks. A 2024 meta-analysis of 8 randomised trials and 472 patients confirmed a significant gain in hair count when microneedling was added. See microneedling for hair loss.

PRP with microneedling

Microneedling is also used to deliver PRP across the scalp in the same session, which makes biological sense and is widely used. Head-to-head trials of this combination are smaller, so we present it as a sensible option rather than a proven superior one.

Where exosomes fit

Exosome-based therapy, such as ExoRevive, is a newer regenerative option that can be added to PRP. Its evidence base is earlier and smaller than PRP's, and we explain that before anyone decides. It suits people who want to add a newer option to an established plan rather than replace one.

Starting a combination safely

Diagnosis comes first, because combining treatments for the wrong cause doubles the cost without helping. Start minoxidil knowing an early shed is common and usually temporary, as explained in the minoxidil shedding phase. Do not apply minoxidil immediately after microneedling unless your doctor advises it, because absorption increases. Low-dose oral minoxidil is a prescription treatment that needs a doctor's assessment, explained in oral minoxidil. Minoxidil and many hair medicines are not suitable in pregnancy or while breastfeeding.
