What is causing your shoulder pain?
Rotator cuff pain
Pain on the outer upper arm, worse lifting the arm to the side or overhead and when lying on that side at night. Frozen shoulder

A tear after an injury
Sudden weakness after a fall or heavy lift, especially an inability to raise the arm, can mean a full-thickness tear. In an active person this needs a shoulder surgeon promptly, because early repair gives the best chance.
Pain from elsewhere
Pain that spreads below the elbow with tingling often comes from the neck. Pain on the top of the shoulder may come from the small joint between the collarbone and shoulder blade.
The options for rotator cuff pain
Steroid
What it is. A corticosteroid injected into the space above the rotator cuff under ultrasound guidance.
What the evidence shows. It relieves pain faster than PRP. A 2026 trial of 107 patients with partial-thickness tears found steroid gave better pain relief at 3 and 6 months. Repeated injections may weaken tendon tissue, so they are best limited.

PRP
What it is. Platelet-rich plasma from your own blood, injected into the damaged area of tendon under ultrasound guidance.
What the evidence shows. In the 2024 double-blinded trial by Rossi and colleagues, 1 PRP injection outperformed steroid at 12 months for pain and function, with treatment failure of 12 percent against 30 percent. A 2025 meta-analysis of 15 randomised trials found better shoulder function with PRP at 12 and 24 weeks. We grade the evidence as favourable, not settled.

Surgery
The CSAW trial, led from Oxford and published in The Lancet in 2018, randomised 313 patients with long-standing impingement pain to keyhole decompression, placebo keyhole surgery without decompression, or no surgery. Decompression gave no meaningful benefit over the placebo operation. Surgery remains the right choice for a repairable full-thickness tear, but not for most tendon pain.

The options for frozen shoulder
Steroid
In the early, painful phase, a steroid injection into the joint often eases pain and helps sleep.
Hydrodilatation
A larger volume of fluid, often with steroid, is injected into the joint to stretch the tight capsule. A 2023 meta-analysis of 39 studies and 2,623 participants in the British Medical Bulletin found greater short-term improvement in disability and outward rotation than with steroid injection alone, though the authors noted the clinical relevance is not yet clear. PRP
A 2026 meta-analysis of 13 randomised trials and 1,056 patients in Frontiers in Medicine found similar pain relief to steroid at 1 and 3 months, and better pain relief, function and movement with PRP at 6 months. The authors suggest steroid for the early painful phase and PRP for the later stiff phase.
