Where is your hip pain?

Groin pain

Pain deep in the groin, often with stiffness and difficulty putting on socks or getting out of a car, usually comes from the hip joint. In people over 45 the most common cause is osteoarthritis. In younger, active people, a tear of the labrum, the rim of cartilage around the socket, or a shape mismatch of the joint called femoroacetabular impingement can be responsible.

Outer hip pain

Pain on the outer side of the hip, worse when lying on that side, climbing stairs or standing on one leg, is usually gluteal tendinopathy rather than bursitis. It is most common in women aged 40 to 60. Our full guide is gluteal tendinopathy.

Buttock pain

Pain in the buttock, sometimes spreading down the back of the thigh, often comes from the lower back or the sacroiliac joint rather than the hip. See PRP for back pain.

The options for hip osteoarthritis

Steroid

What it is. A corticosteroid injected into the hip joint under image guidance.

What the evidence shows. It can give useful short-term relief, typically for weeks. Repeated injections are best avoided, and surgeons usually prefer a gap before hip replacement surgery.

Hyaluronic acid

What it is. A lubricating gel injected into the joint.

What the evidence shows. NICE guidance states that hyaluronic acid injections should not be offered for osteoarthritis, because the evidence of benefit is weak.

PRP

What it is. Platelet-rich plasma prepared from your own blood and injected into the joint under ultrasound guidance.

What the evidence shows. A systematic review of 5 randomised trials found all reported reduced pain and improved function, with no major adverse events. The options for outer hip pain

Exercise first

In the LEAP randomised trial, published in the BMJ in 2018, an education and exercise programme outperformed a single steroid injection and a wait-and-see approach at both 8 and 52 weeks. By 52 weeks, roughly 80 percent of the exercise group had recovered, against around 50 percent in the other groups.

Steroid

Steroid can ease pain briefly, but the benefit is shorter-lived than exercise and repeated injections may weaken the tendon. It is not a first choice.

PRP
